What is Telfast?

Telfast is a selective H₁-receptor antagonist that alleviates the hallmark symptoms of allergic rhinitis, such as sneezing, itching, nasal congestion, and watery eyes, by competitively inhibiting histamine H₁-receptors in peripheral tissues. It also soothes chronic idiopathic urticaria by reducing wheal formation and pruritus. Its second-generation status denotes an improved safety profile, with negligible penetration of the blood-brain barrier that avoids sedation, allowing patients to maintain normal daytime functioning.

First launched in 1996 after extensive clinical evaluation, fexofenadine’s molecular structure was designed to limit central nervous system ingress, making it safer than many first-generation antihistamines. Its favourable pharmacokinetics, rapid absorption, 24-hour duration, and renal clearance mean consistent symptom control with minimal risk of drug accumulation in most patients.

What is Telfast Used For?

Population: Adults and adolescents aged 12 years and above.

Indication: Relief of sneezing, rhinorrhoea (runny nose), nasal congestion, itching of the nose and throat, and itchy, watery eyes triggered by pollen, mould spores, and other outdoor allergens.

Dosing: One 120mg or 180mg tablet once daily, preferably at the same time each day to maintain steady blood levels.

How Does Telfast Work?

Absorption: Rapid oral absorption with Tmax around 2–3 hours post-dose.

Mechanism: Competitive inhibition of peripheral histamine H₁-receptors, preventing histamine-mediated vasodilation, increased vascular permeability, and sensory nerve stimulation.

Pharmacokinetics: Minimal hepatic metabolism; primarily excreted unchanged via the kidneys.

Pharmacodynamics: Effective receptor blockade throughout the dosing interval, providing consistent symptom relief.

Can Telfast Be Purchased Over the Counter?

In the UK, Telfast 120 mg and 180mg remains prescription-only (POM).

Always consult a GP or pharmacist to confirm the appropriate formulation and dosing regimen for your specific condition and health profile.

Alternative Medications

If Telfast is unsuitable or ineffective, other non-drowsy antihistamines such as loratadine or ceterizine may be recommended. Nasal sprays and eye drops can also be used for targeted symptom relief.

Dosage & Administration

  • How Much: One tablet
  • Who: Adults
  • How Often: Once daily
  • When: Preferably at the same time each morning, before or after food depending on patient preference.
  • Tip: Taking it with breakfast may reduce any mild stomach discomfort.

Administration Tips

Swallow Whole: Do not chew, crush, or split film-coated tablets to preserve controlled absorption.

Consistency: Take at the same time every day to maintain steady blood concentrations.

Missed Dose:

If you remember within 8 hours, take it as soon as possible.

If more than 8 hours have passed, skip the missed dose and resume normal schedule.

Do not double up to catch up.

Exceeding Dose: In case of accidental double intake, monitor for symptoms (e.g., dry mouth, headache) and seek medical advice if severe.

Storage

  • Store tablets at a controlled room temperature of 20–25 °C.
  • Protect from moisture—keep in original blister pack until use.
  • Keep out of reach of children.
  • Dispose of any unused medicine after the printed expiry date on the pack.

Microcrystalline Cellulose: Binder, aids tablet strength.

Magnesium Stearate: Lubricant, prevents sticking in manufacturing.

Povidone (Polyvinylpyrrolidone): Binder, enhances disintegration.

Silicon Dioxide: Glidant, ensures uniform powder flow.

Film Coating:

- Hydroxypropyl Methylcellulose (HPMC): Film-forming agent.
- Titanium Dioxide: Opacifier.
- Macrogol 400 (Polyethylene Glycol): Plasticiser for film flexibility.

Allergen Notice: Contains lactose – patients with rare hereditary problems of galactose intolerance should consult their GP. For a full list of excipients, refer to the Patient Information Leaflet.

Common (≥ 1% of users)

Headache: Typically mild; resolves without intervention.

Drowsiness (rare, mild): Less common than with first-generation antihistamines.

Nausea: May occur if taken on empty stomach; food can mitigate.

Dizziness: Usually transient and mild.

Fatigue: Rare and often related to an underlying allergy rather than a medication.

Uncommon (0.1–1%)

Dry Mouth: Simple measures (sipping water, sugar-free lozenges) help.

Palpitations: Rare; monitor if you have pre-existing cardiac issues.

Abdominal Pain: Mild cramping; drink fluids and avoid heavy meals.

Back Pain: Transient, typically unrelated to dosing schedule.

Rare (< 0.1%)

Hypersensitivity Reactions: Rash, pruritus, angioedema—discontinue immediately and seek urgent care.

Tachycardia: Rapid heart rate; report if accompanied by chest pain.

Paresthesia: Tingling sensations; generally reversible upon discontinuation.

If severe or persistent side effects occur, discontinue use and seek medical attention, contact your GP, NHS 111, or attend A&E to ensure prompt management and prevent complications.

  • How quickly does Telfast work?

Relief typically begins within one hour of oral administration, with full effect by 2–3 hours post-dose.

  • Can I drive after taking Telfast?

Telfast is non-sedating in the vast majority of users, but individual responses vary. If you experience dizziness or drowsiness, avoid driving or operating machinery.

  • Can Telfast be taken with other medications?

Avoid antacids containing aluminium or magnesium within two hours before or after taking Telfast, as they can reduce absorption. Always tell your prescriber about all concurrent medicines, including herbal supplements.

  • Is Telfast suitable during pregnancy?

Limited human data exist. Use only if clearly needed and after consulting your GP. The benefits should outweigh potential risks.

  • Can children take Telfast?

Licensed for use in adolescents 12 years and older. For younger children, specialist paediatric advice is required, and lower-strength or liquid formulations may be recommended off-label.

  • What if I miss a dose?

Take it as soon as you remember, unless it’s almost time for your next dose. Do not take two doses to make up.

  • Does Telfast cause weight gain?

No evidence links fexofenadine to changes in body weight.

  • Can I drink alcohol with Telfast?

Alcohol can enhance any mild sedative effects. Best avoided, especially if you notice dizziness or drowsiness.

  • Where can I buy Telfast?

Available by prescription at UK pharmacies. Some lower-strength fexofenadine products (e.g., 60 mg) may be available as Pharmacy-Only (P) for milder symptoms.

  • What sets Telfast apart from other antihistamines?

Its rapid onset, 24-hour duration, and minimal sedation make it a preferred choice for adults and adolescents needing reliable, all-day allergy relief without cognitive impairment.

  • Who to contact if symptoms persist?

If symptoms remain uncontrolled after 3–5 days of consistent use, consult your GP or pharmacist for reassessment, possible dose adjustment, or alternative therapy.
